Brain Fog That Won't Lift? A Clyde North Chiropractor on the Structural Cause Nobody Is Checking
There's a pattern I keep seeing in my clinic.
Someone comes in frustrated, exhausted, and a little embarrassed. They can't explain why their head feels cloudy all the time. They've done the sensible things. Cut back on caffeine. Improved their sleep routine. Had blood work done. Everything came back normal.
Their GP used words like "stress" or "burnout." Nothing wrong on paper. But the fog hasn't lifted.
What these people almost always share is a history of years at a desk or hours on a phone. Many have an old injury that never fully resolved. And when I look at their spine, I consistently find the same thing. A neck that has lost its natural curve, and a head that has drifted forward.
That isn't a coincidence. And it isn't a metaphor. It's a mechanical change that may be quietly affecting how their nervous system works.
This isn't an article about sleep hygiene or supplements for focus. This is the conversation I have when someone has tried all of that and still can't think clearly.
Why the usual advice keeps falling short
The standard response to brain fog is to treat it as a brain problem. Better sleep. More water. Mindfulness apps. Magnesium.
These aren't wrong. But they're incomplete if there's a structural issue underneath.
Your nervous system isn't just your brain. It's the entire communication network running from your brain, down through your spinal cord, and out to every part of your body. When something disrupts that network physically, the brain receives less clear signals.
The result can show up as fatigue. Poor concentration. A blunted sense of alertness. That frustrating inability to think clearly even though nothing is "wrong."
No sleep app fixes a mechanical problem.
The neck connection most people never hear about
Here's where it gets specific.
Your neck has a natural inward curve called the cervical lordosis. That curve isn't cosmetic. It protects the spinal cord and the blood vessels that supply your brain.
When that curve flattens or reverses, which is very common in people who spend years looking at screens, two things can happen. The veins that drain blood from the brain can become compressed. And the nerves running through the upper neck can come under sustained pressure.
A 2026 study published in Frontiers in Neurology looked at 227 patients presenting with brain-based symptoms including brain fog, difficulty concentrating, anxiety, and depression. Every single patient had forward head posture on imaging. 90% had a reduced cervical curve. And 98% showed compression of the internal jugular vein, which is the main drainage pathway for blood leaving the brain.
The researchers noted that when this drainage is compromised, pressure can build inside the skull. That pressure has been linked to a range of symptoms including mental confusion, difficulty concentrating, and brain fog.
Think of it like a drain that's partially blocked. Water still flows, but it backs up. Your brain works the same way. If the blood can't drain efficiently because the neck structure is compressing the pathway, pressure builds and cognitive function suffers.
I want to be clear about the limits of this research. This was a cross-sectional study showing association, not causation. It doesn't prove that fixing the neck cures brain fog. But it does show a consistent structural pattern in people experiencing these symptoms, and it points to a mechanism that makes biological sense.

The posture pattern behind it
The structural change driving this isn't random. It follows a predictable pattern.
Years of sitting, driving, and looking down gradually flattens the cervical curve. The head drifts forward. The load on the upper neck increases. A study pooling 13 pieces of research found a strong link between a reduced spinal curve and pain. And a separate study found that loss of the neck curve was present in roughly 42% of people with neck pain.
The upper cervical spine sits right at the junction of the brainstem. Sustained mechanical stress in that area doesn't just cause neck pain. It can affect the nerve pathways and blood vessels that supply the brain itself.
This is the connection most people have never been shown. The brain fog, the neck pain, the headaches, and the fatigue aren't necessarily separate problems. They may be different expressions of one structural issue.
What happens if you keep waiting
This part isn't meant to alarm you. But it's worth being honest.
Spinal structure doesn't tend to drift back on its own. Without help, the postural patterns driving the problem typically continue or get worse.
The joints and discs in the neck can begin to show wear over time. Research has shown that loss of the cervical curve is directly linked to how quickly the discs degenerate.
The nervous system compounds it. Pain and dysfunction that persist change how the brain processes signals. The longer the pattern runs, the harder it becomes to unwind.
People often describe losing ground on tasks that used to feel easy. Concentration shortens. Mental stamina drops. The fog becomes the new normal.
The hopeful side is that spinal structure can be improved. The nervous system responds well when mechanical load is reduced. Many of the people I work with describe improvements in clarity, energy, and that general sense of feeling more like themselves as their structure improves. Those outcomes aren't guaranteed and they vary from person to person. But the biology makes sense.
How we assess it
When someone comes in describing brain fog, we don't guess, we measure.
The approach we use is Chiropractic BioPhysics (CBP). It's built around measuring the actual shape of your spine and identifying where the curves have changed.
The assessment includes a postural analysis and a thermography scan to check where the nerves along your spine are under stress. Where needed, we take X-rays to measure the angles of your cervical curve. This tells us whether the structure of your neck is part of the picture.

A review of controlled trials using CBP methods found that patients who received cervical traction increased their neck curve and held that correction at both three months and two years. The control group showed no structural change at all.
I want to be honest. Not every presentation of brain fog has a spinal cause. The evidence linking cervical structure to cognitive symptoms is emerging, not established. But when someone has tried everything else and their spine shows a clear structural change, it's a connection worth investigating.

One thing you can try right now
Set a timer for two minutes. Sit in whatever position you default to at a screen. Then check three things.
Where is your chin relative to your chest? Are your shoulders rounding forward? Does your lower back have any curve at all?
Most people find their chin is jutting forward, their shoulders are rolled in, and the lower back curve has collapsed entirely. That's the posture pattern that, held for hours daily over years, creates the structural load I've described.
You can't undo that pattern with awareness alone. But noticing it is the first step. Pull your chin back so your ears sit over your shoulders. Briefly restore your lower back curve. Hold it for 60 seconds.
It's not a fix. But it gives your nervous system a brief break from the load. It may also be the first time you've noticed how far forward your head actually sits.
Is this the right path for you?
Not everyone with brain fog has a spinal cause. Brain fog can overlap with thyroid issues, low iron, sleep disorders, and other conditions that sit outside chiropractic scope. A structural assessment doesn't replace a GP workup. If you haven't had one, start there.
But if you've done the medical checks and still have no clear answer, a structural assessment may offer a different angle. This is especially true if your symptoms include mental fatigue alongside neck tension, headaches, or disrupted sleep.
