The Bulging Disc on Your Scan Isn't the Whole Story: Honest Sciatica Treatment in Clyde North

There's a scene that plays out in my clinic almost every week.


Someone walks in holding a folded MRI report like it's a death sentence. They slide it across to me, point to a word, herniation, degeneration, protrusion, and say some version of the same sentence: "So that's it, isn't it? My spine's stuffed."


I understand why they feel that way. When you're in pain and a scan hands you a frightening word, it's natural to assume the word is the problem. But here's what I've learned treating sciatica for years, and what almost no one tells you when they hand you that report: the scan is rarely the whole story, and sometimes it's barely part of it.


Consider this. When researchers reviewed imaging of more than 3,000 people with no back pain at all, they found disc bulges in nearly a third of people in their twenties who felt completely fine, climbing to 84% by age 80. Signs of disc degeneration showed up in more than a third of people that age who had no symptoms whatsoever. In other words, the very findings that terrify people are extremely common in people who feel perfectly fine. A scary word on a report is often just a photograph of ageing, not the cause of your pain.


I want to walk you through what I actually see in practice, including a few things that contradict the advice you've probably already been given. This isn't the standard "what is sciatica, here are seven stretches" article. You can find a thousand of those. This is the conversation I have with patients when they sit down across from me.


Why "sciatica" is the most misused word in my clinic


Let's start with the word itself, because it causes more confusion than almost anything else.


"Sciatica" isn't actually a diagnosis. It's a symptom, pain that travels along the path of the sciatic nerve, usually down one leg. Saying you have sciatica is a bit like saying you have a fever. It tells me something is going on, but it doesn't tell me why.


True sciatica, what we call lumbar radiculopathy, happens when a nerve root in your lower back is compressed or irritated, most often at the L4/L5 or L5/S1 level. But a great deal of the leg pain I see labelled "sciatica" isn't that at all. Sometimes it's referred pain from a joint. Sometimes it involves the deep gluteal muscles or the piriformis. Sometimes it's a nervous system that has become oversensitised after months of guarding and bracing.


This matters enormously, because the cause dictates the fix. Treating gluteal referred pain as though it's a compressed disc, or the other way around, is how people end up doing the wrong rehab for months and wondering why nothing changes. Getting the diagnosis right on day one is, in my experience, the single biggest factor in whether someone actually gets better. If you want to understand the broader picture of what can drive lower back pain, it's worth knowing that the leg is often just where the symptom shows up, not where the problem lives.

What the textbooks get right, and what recovery actually looks like


Open almost any health website and you'll read a reassuring line: most sciatica resolves within six weeks. And to be fair, that's often true. A lot of acute sciatica does settle with time, movement and patience.


But I think patients deserve the fuller picture, because that tidy statistic doesn't match what I see in the room.


A large French study following over 600 people found that 55% still reported sciatica symptoms two years later. The strongest predictors of persistence weren't anything you'd read on a scan. They were things like driving more than two hours a day, repeatedly carrying heavy loads, and high levels of stress. In other words, what your body goes through on a daily basis matters as much as the disc.


I tell people this not to frighten them, but because false reassurance is its own kind of harm. If you've been told "it'll just go away" and it hasn't after three months, you're not broken and you're not imagining it. You may simply have one of the presentations that needs active, targeted help rather than more waiting amd "resting".


Why the stretches you found online sometimes make it worse


This is the part that surprises people most.


When sciatica strikes, the instinct, and the advice from every listicle, is to stretch. Toe touches, knees to chest, aggressive hamstring stretches. And for some presentations, gentle movement genuinely helps.


But if your leg pain is being driven by a posterior disc bulge, repeatedly bending forward can increase pressure on the back of that disc and push your symptoms further down the leg. There's a concept we use clinically called centralisation versus peripheralisation: broadly, movements that draw the pain back toward your spine tend to be helping, while movements that send it further down the leg tend to be aggravating things.


The generic "stretch it out" advice ignores this completely. It assumes every sore back is the same back, caused by the same problem. I've met plenty of people who were diligently doing the exact stretches that were making their disc injury worse over time, and blaming themselves for not trying hard enough. The problem wasn't effort. It was direction.


The thing almost no one tells you: your disc can heal


Here's the part I wish every frightened person holding an MRI report could hear first.

Discs are not static. They are living tissue, and they have a real capacity to recover. When researchers pooled together multiple studies on disc herniation, they found that herniated discs spontaneously resorbed, literally shrank and were reabsorbed by the body, in around two thirds of cases.


And here's the counterintuitive twist: the largest, angriest looking herniations on a scan often resorb the most reliably of all. The scary report is frequently the one with the best natural prognosis.


So when someone shows me imaging and assumes the worst, my honest answer is usually that this is far more hopeful than they think. The body is remarkably good at healing itself, if we give it the right mechanical environment to do so. And that's exactly where my approach comes in.


Sciatica Treatment in Clyde North: What Recovery Actually Looks Like


Once we've confirmed that your leg pain is genuinely nerve compression coming from the disc, my goal is simple: Take pressure off the nerve, and restore the conditions your spine needs to recover.


The two tools I lean on most are spinal decompression therapy and Chiropractic BioPhysics (CBP).


Spinal decompression uses a specialised, motorised table to apply gentle, cyclical traction to the spine. The aim is to reduce pressure inside the affected disc, ease the compression on the irritated nerve root, and improve the flow of fluid and nutrients into the disc, supporting the body's own resorption process. A randomised controlled trial of decompression added to standard physiotherapy found significantly greater improvements in pain, disability and function than physiotherapy alone, with the authors concluding it was both statistically and clinically more effective.


CBP is the structural half of the equation. Sciatica that keeps coming back is often sitting on top of an underlying postural problem, a lost curve in the lower back, altered loading through the spine, that no amount of symptom chasing will fix. A controlled trial using extension traction to restore the lower back's natural curve in patients whose leg pain was coming from the disc found significantly better outcomes in the treatment group, and importantly, those advantages were still present at the six month follow up. Addressing the structure, not just the symptoms, is what gives people their best shot at long-term change rather than a revolving door of relapses.


I want to be transparent about the evidence: these are promising studies, not magic. Results vary from person to person, and decompression isn't right for everyone. But for the right candidate, someone with confirmed sciatica coming from the disc, it's one of the most effective tools I have that doesn't involve surgery. Pain is not just mechanical


Here's the part I'm always honest with people about, because many people tend to leave these issues untreated until they get bad enough to become genuinely debilitating.


When a nerve stays compressed for long enough, the problem stops being purely mechanical. Pain that persists changes the nervous system itself: the pathways carrying those signals grow more sensitive, and the brain effectively gets better and better at producing pain. We call this central sensitisation. The longer pain runs, the more the system learns it, and the harder that is to unwind.


In severe cases, sustained pressure on a nerve can cause lasting changes in how it works, including significant weakness in the muscles it supplies, and in rare instances, changes affecting bladder and sexual function.


Then there's the spiral almost no one notices. Pain makes you guard, guarding makes you move less, and moving less weakens the very muscles that support your spine, which feeds the problem. Add the natural fear of "doing more damage," and you have a loop that research links to slower recovery and chronic pain.


None of this is meant to frighten you. It's meant to encourage you to act early enough to give your body its best chance of recovery before the problem becomes chronic. Acting while the problem is still mechanical is one of the biggest things within your control.
